Infections bactériennes

Principaux germes

  • Staphylococcus epidermidis
  • Staphylococcus aureus
  • Haemophilus influenzae
  • Streptocoque
  • Pseudomonas aeruginosa

Traitement

Antibiothérapie systémique:

  • Ofloxacine PO 200 mg x 2/j
  • + Imipenème IV 1 g x 3/j

          OU

  • Ofloxacine PO 200 mg x 2/j
  • + Pipéracilline IV 4 g x 3/j

Collyre fortifié:

  • Vancomycine 50 mg/ml
  • + Ceftazidime 20 mg/ml

Injection intravitreenne :

  • Vancomycine 1 mg dans 0,1 ml
  • + Ceftazidime 2,25 mg dans 0,1 ml

Durée

  • Antibiothérapie systémique: 2 j IV et 3 j PO
  • Collyre fortifié: Instillation horaire pendant 2 j puis toutes les 2h

Infections fungiques

Candida spp

Aspergillus

1er choix

  • Amphotéricine B IV
  • + Amphotéricine B intravitreén 0,001-0,005 mg dans 0,1ml
  • Collyre amphotéricine B 0,7-1 mg/kg/j 2,5 mg/l, 1 goutte /h

Alternative

  • Voriconazole 6 mg/kg/12h à J1 puis 4 mg/kg/12h
