Si hospitalisation (doute diagnostique, signes généraux important, co-morbidité, contexte social):
Penicilline G 12 MU/j en 4 à 6 fois, relais amoxicilline 3 à 4,5g/j en 3 prises OU penicilline V 3 à 6 MU/j en 3 prises

Si à domicile: Amoxicilline 3 à 4,5 g/j en 3 prises

Alternative

  • Pristinamycine 1g x3/j PO

          OU

  • Clindamycine 600 mg x 3/j

Durée

  • 7j

Prélèvement

  • Non (+/- hémocultures)

Mesures particulières

  • Contre indication des AINS
  • Pas d’anticoagulant préventif en systématique sauf si facteurs de risques thromboemboliques
  • Traitement de la porte d’entrée et des facteurs favorisants: lymphoedème, insuffisance veineuse
